Health is the amount of damage a player can take before death. All players will start out with 100 HP (100%) unless certain modifiers are active, which mostly decreases max health or initial health. The player's health bar normally remains invisible until health is below 100% or if the player has any star shield.
The player's default maximum health is 100 HP, this value can vary through multiple ways.
Star shield, also known as overheal, refers to the extra health that a player can gain beyond their normal max health. Star shield does not increase the player’s maximum health but acts as a buffer against incoming damage. The max amount of star shield is always 100.
Health transfers when teleporing between floors. If the player had less than 25 HP (or was dead) when teleporting, it will be set to 25 HP. Star shield does not transfer between floors, players will always spawn with 0 star shield, regardless of the amount previous to the teleport.

Death screen
When a player's health reaches 0 (or 1), they will die, leading them to the death screen. This comes with a unique message from Guiding Light (or Curious Light if the player died in a subfloor) about the entity or hazard that killed them.

- Prior to The Hotel+ Update, vitamins used to have a passive health boost effect for a short amount of time, but afterwards, the effect was removed.
- This effect was removed so they could add separate healing items in the future.[1]

- Before The Hunt Update, when teleporting between floors, the health bar didn't appear even if the player had under 100 HP. It would only appear again if the player took damage or healed in any way.
- In SUPER HARD MODE!!!, star shield doesn't exist (nor the items that grant star shield), as this mode released before its implementation, however, a very similar feature exists, known as shield.
- This, together with the 2 items that grant the shield (Mini Shield Potion and Shield Potion), is a reference to the game Fortnite.
- Shield works exactly as star shield. Maximum shield is 100. Unlike most ways to obtain star shield, it doesn't decay slowly, the only way it can decrease is taking damage.
- In "SUPER HARD MODE!!!", when consuming any shield potions the health bar will remain visible, alongside with the shield bar.
- However, when healing to max health while the shield bar is visible, the health bar will disappear while the shield bar remains visible. This is likely to be a bug.
- Health won't go below 2 HP if the first door of the floor was never opened. The only ways to die are dying to an instakill entity or hazard (Dread, Floods, Mandrake, Figure, A-120, etc.)
- This system is the same used for Timothy, Snare and Gloombat Eggs, non-lethal entities and hazards.
- As of The Halloween Spotlight Event players can know their MAX HP (unless it is higher than 100) looking at their health bar.
- When The Hotel+ Update came out, the Green Herb passively healed the player. Since The Mines Update, it is not passive regeneration and instead heals the player by 10 HP every time the player opens the door. Since the Mines nerf, the Green Herb heals by 5 HP every time the player opens a numbered door.
- It is unknown why 1 HP is fatal for the players, as in most games 1 HP is still alive while obviously 0 HP is lethal, it could possible be a bug and isn't fixed due to insignificance.
