Items are useful tools found throughout any run of any floor. They can offer various advantages to the user, such as making dark rooms easier, boosting movement speed or getting rid of certain entities temporarily. They do not save the next time you enter a run (unless you placed said item in a rift, or you go directly into another floor) and can only be used in the run you bought/found them in.
They are obtained through containers, such as drawers. Once you find an item and pick it up, it will be added into your inventory. Additionally, some items can be purchased through the Pre-Run Shop, bought at Jeff's Shop or by bartering with Louie by dropping an item to get a new random item, or several new items. They can also be obtained within the Admin Panel.
The items' appearance/design can be also changed with Item Skins.

Notes
- "Major Items" are items that can spawn naturally, can be given by Louie in return of a traded item, can be transformed in a Rift, or can be bought at any shop.
- "Minor Items", sometimes referred as "Essential Items" are items that don't take space in the inventory, can't be transported across floors (with the exception of the Lotus Petal), can't be dropped, and can't be stored to Rifts.
- "Cosmetic Items" are items that are only for cosmetic purposes and do not have any impact on gameplay.

Trivia
- There are currently 67 items that can be found in-game on all floors.
- There are 33 major items, 10 minor items, 1 cosmetic item, 4 resources, 2 secret items, and 17 modifier/event-exclusive items as of October 27, 2025.
- Dropped items/items found in containers emit a small white light.
- The attribute for minor items is "CannotTransport".
- This is presumably referring to the fact that minor items cannot be "transported" in Rifts.
Bugs
- For some unknown reason, when using either "More Stuff", "Less Stuff", or "Out Of Stuff", it cannot be used with "Wear And Tear" modifier.
- This might've been done because of "Out Of Stuff", since the player can't find items, it's most likely for them to carry none through the run, meaning it would be a free 5%.
- Even if the player purchases items at shops, no resources can be found for them.
- Another possible quirk is that when using "Small Hands" or "Baby Hands" modifier, it cannot be used with "More Stuff", "Less Stuff", or "Out Of Stuff" as well.
- It could posibly have the same reason as "Wear And Tear" modifier
- In RANKED MODE, Throwable Props can bypass the inventory limit. Picking up other item while holding a prop will replace that slot with the item, being able to have more items than the intended limit. This can be repeated multiple times.
- Unequipping and equipping the same item very fast simulates lag, that makes the server think the player is in the same place until the simulated lag stops, despite visually moving in all clients. This bug has been present since the release.
- In the main build of the game, the anti-cheat will try to teleport the player back to where the simulated lag started; this doesn't happen in other builds like SUPER HARD MODE or Hotel-, as this kind of anti-cheat is not present in those builds due to switching items with the mouse's scrollwheel not beeing present.
- In The Mines Update, items received a big hitbox that could block ray-casting from entities (Rush, Ambush, A-60, A-120, Blitz), saving players succesfully by just staying in the middle of an item inside any open container or table or naturally on the floor in The Rooms. This was patched in The Content Update, as their hitbox don't block ray-casting anymore.
- Items dropped by players had a much smaller hitbox that couldn't save players from entities.
